ELIGIBILITY: To be elected to the Delaware Sports Hall of Fame, the person must be judged to have brought lasting fame and recognition to Delaware through his/her contributions in athletics or have been outstanding within the state's borders. A candidate must have a clear connection with Delaware before or during his/her period of excellence.

This questionnaire must be completed on behalf of each candidate for induction into the Delaware Sports Hall of Fame. A summary narrative and additional information (news clippings, etc.) should be attached when appropriate.
